The FreeStyle Libre 2 system is covered by the Department of Defense TRICARE for those who qualify. Please contact your physician as a prescription is required for FreeStyle Libre 2 readers and sensors.
Prescription Requirements1 :
- Beneficiary must complete a comprehensive diabetic education program
- Treatment regimen including at least 3 injections per day or insulin pump therapy, with frequent self-adjustment of insulin doses in the last three months (except for gestational diabetes, which has no time requirement for the self-adjustment of insulin)
- Beneficiary has documented blood glucose self-testing on average of at least 4 times per day
- Beneficiary must have ANY of the following:
- Glycosylated hemoglobin level (HBA1c) greater than 7.0% or less than 4.0%,
- OR history of unexplained large fluctuations in daily glucose values before meals,
- OR history of early morning fasting hyperglycemia ("dawn phenomenon"),
- OR history of severe glycemic excursions,
- OR hypoglycemic unawareness,
- OR history of recurrent, unexplained, severe hypoglycemic events (i.e. blood glucose less than 50 mg/dl),
- OR history of recurrent episodes of ketoacidosis,
- OR hospitalizations for uncontrolled glucose levels,
- OR frequent nocturnal hypoglycemia,
- OR the beneficiary is pregnant and has poorly controlled diabetes or gestational diabetes
